Please complete if you have any interest in your child being a part of After Care for 2022-23 School Year. You only need to complete the form ONCE.(A Small Convenience Fee is Added for Credit Card Payment, Debit/Credit only, NO Cash or Money Orders, weekly fee charged automatically every Monday through the end of the program)
